has anyone ever discharged a patient with their IV still in?

2 I discharged a patient and forgot to d/c the IV. I feel like an idiot. Am I the only one?
I've left a hep lock in........

Nope... left a heplock 2 times! Once they called and I explained about removing it (because they refused to return), the second came back and I removed it!

No, you are by far not the only one. When I worked as a Paramedic we got lots of calls to pull out hep traps left in by hospitals. Don't feel like an idiot it happens

I have not done it, but I've had patients call after discharge and ask what to do about it because someone else did! It happens frequently.

i've seen it happen a number of times. Patients are usually in such a rush to get back to their homes and end up forgetting it's even there.

personally no, unless it was meant to stay in ( for community IV pathway) but have had colleagues who have ... it's not a big deal - ask them to return to the unit / nearest ED or send Community nursing / Ambulance responder to pull it at home ... then be ready for the paperwork stuff it is a clinical incident after all,

Did it on orientation. Patient was in a hurry to get d/c the INT was still in, I realized that preceptor didn't do it and I didn't either. I called the pt. they acted ticked but explained that we forgot it and how to take it out. Will they tell someone else, yeah probably. Pt was also prob trying to hide it due to drug use. Really? They got dressed on their own.

I haven't done this yet, but I believe we have had one pt, who refused to let his nurse take out his IV when he was dc'd. Idk how that story ended, i.e. if it had to be let in.
