Previous community nurse seeking advice on switching work environments

Hi everyone!
I am a young LPN, and have spent the last two years working as a community palliative care nurse. I have recently moved to a new province, and am interviewing next week for a hospital job.
My previous job gave me a lot of independence. We practiced with a primary care model, where, with collaboration with the other team members, I would oversee most of the patient's care from diagnoses to death, even while they were in hospital for treatments/stays. I was often the eyes and ears of the physicians/case managers etc, and the main contact and advocate for my patients and their families.
Any advice on transitioning from community visiting nursing to a hospital environment would be greatly appreciated!
