Assessment Sources

Does anyone have a good online source for assessment information?

For example, for pain, it would look something like this:

• location
• intensity
• quality
• aggravating factors
• alleviating factors
• onset
• duration

I would like to find something similar for different diseases/disorders (DM, HTN, COPD, etc...whatever I can get). I would prefer something free, but if you have a book you think would be helpful, I'd like to hear what it is.

It's a little advanced but I have yet to find a book that's better...I have the 3rd edition by Barbara Bates but it's all good information.
