G-Button documentation

Hi I'm just wondering whats proper documentation for g-button. Usually I document whether it's clean, dry, intact, any redness, or shows if there are signs of infection. I had a pt whose physician applied silver nitrate to the area yesterday and this am there was dry drainage brown in color around the area. After cleaning there was no reddness or signs of infection. So just wondering how you all would properly document that? Thanks
"Dry brown drainage in area around g-button. Cleaned with _________. After cleaning, no redness, discoloration, or signs of infection."
