Discipline for low pain reassessment scoresRating: (votes: 0) Our charting gets audited quite a bit for pain reassessment. It's very annoying. I could assess and re-assess indefinitely with some of our more chronic patients. Comment:
Lol ours wants 100%. one of the major things they check (after restraint documentation).

JHACO requires every pain med administration have a follow up assessment for effectiveness. We are expected to reassess 100% and chart it. No exceptions.

Compliance with these issues and getting good scores is directly tied to reimbursement these days. Think of it as an investment in your next raise. It is a real pain in the behind and REALLY ANNOYING!!!!! But important.

Everywhere that work is serious about pain reassessment documentation. It's annoying but understandable. I like the charting systems I use where there is a reminder on the MAR to reassess pain so if you forget it shows up on the MAR the same way an overdue med would. I worked at one place that used EPIC, which I love but there was no pain reassessment reminder. They would do chart audits and everyone kept getting in trouble for forgetting the reassessments, that's just annoying.

I use epic at work. I have mentioned for a year that other hospitals I have been too or know people who work there tell me about the pop up which is nice. I do understand but sometimes it gets so crazy, as I am sure we all can agree to, things jut slip my mind. Plus with management complaining about all this overtime we have. I am just so frustrated.

Yes we have 100 percent compliance expectation. You need to think of it as providing good nursing care...you have assessed for pain.....provided an intervention and now you need to reassess and see if your nursing intervention was effective. What I used to do was to put a check box next to the the time I gave the pain RX and once I documented the pt's response I put a check in the box. FYI : It's The Joint Commission (TJC) no longer JCHAO. That was changed many years ago.
