IM injection with clean aspiration but blood in needle after injection

So just like the topic says I am a fairly new nurse and I work in a doctors office. I have given a lot of IM injections and sometimes I will find blood in my needle after the injection. I always aspirate and have never had blood return. Is this blood in the needle from when I am pulling out, and does it mean I'm doing something wrong?
Blood in the needle or syringe? When pulling the needle out, you either kinked a vein or went through one. You're doing good, keep aspirating.

There was no blood in the syringe where the medication is supposed to be, but there is blood where the needle and syringe twist together.
