485 plan of care for home health

Does anyone know if an LVN can fill out the care plan (485) form that is faxed to the physician or if an RN must do this? Also, are there the same 60 day recertification requirements for patients who are on medicaid, or is that just a medicare thing?
I don't know what state you are in. In my state an LPN can not complete a 485, or if she does an RN must sign/co-sign. In other words an LPN can't do a recertification or an admission visit. Again, Medicaid rules may be different in your state. Every patient on home care where I worked had to have a 485 for care, those are your MD orders.
