medicare denials

i am the medicare coordinator at a nursing home in texas, here latley we have been receiving medicare denials to pay for part A and B services, are other facilities recieving these like crazy? could it be a billing error?
Your in-house case manager should be able to follow up on this with CMS for the definitive answer.
