VA Home Based Primary Care Program

Hi Everyone!

I am interested in working for the VA and saw an opening for the Home Based Primary Care program. I have several questions about the program:

-Do staff members visit the patients' homes individually or as a team?
-Does the program ensure that the patients' homes are safe to visit?
-What is the furthest you have to travel?
