24 hr Flow sheets

I'm trying to find something and I hope you all can help. Years ago I did some PRN work for a LTC facility and they had a unique type of unit flow sheets they used that I'm trying find. They were about the size of a large desk calendar and had a carbon sheet underneath. They would list important information and communications on it. Things like who was on antibiotics and why, apt scheduled for the next day etc. This didn't replace report but was there to supplement report. So if anyone can point me in the right direction I would appreciate it.
Contact the facility and ask for a sample?Mind you, it was probably an in-house design.
