NCLEX RE-Examination CA

Hello,

My situation: Graduated 2009 in CA for LVN
Last took my NCLEX in 2011

Since 2011 I've been inactive and have not taken the NCLEX since then. My question is since its now 2015 and I am finally deciding to retake my NCLEX again, do I write to the boards and request the application for "RE-EXAMINATION" for Vocational Nurse Licensure and pay the $150 fee? or Do I have to Re register allllll over again and talk to my Director of Nursing at my school to help me send all the necessary docs. (Live scan, 2x2 picture etc) to the board of nursing as if I was brand new LVN nclex test taker?

I've been calling CA Board of Nursing lvn and emailing them but no response, and I just need a answer.
