JCAHO requires person to person report for transfer of patient care. Is the requirement met when the report is taken by a clerk who fills out a form and puts it in the chart? This is at an accredited facility.
No, you are to give report to the person taking over the care of the patient.

Does this sheet stay in the chart?? I have worked in a place where we recorded report. I hated that. Person to person is much safer IMO.
