Resignation

Hello I just began a new job in a nursing home less than a week ago. I am a new grad and was offered a position in subacute. How do I tell my current manager that I can not work at their facility anymore because I obviously got offered a better opportunity
Give the required notice immediately; just let them know you have accepted another position and thank them for the opportunity.

If you do like the place ask them if you can go PRN if you want and what their policy is with their PRN staff. I often work PRN if its a job I like when I accept a new position, and its a way to make extra money if you need too. LTC is always short staffed. It's also a way to keep a job in your back pocket in case anything happenswith your new job; you already have the interview, physical, and training out of the way.
