OR Specialty Call Teams

We are looking at implementing some specialty OR call teams in our operating room. We are a regional hopsital with 500+ beds and a Level 1 Trauma Center with 24/7 in house coverage. Do you require your specialty teams to be within 30 minutes of the hospital?
Do you pay the specialty team members a premium pay due to them taking more call than others?
The only segregated specialty that takes call in my facility is open heart. Everybody else is expected to be jack of all trades when on call (except they get out of doing hearts). Everyone on call is expected to be not only in the facility but in scrubs and standing in the OR within 30 minutes of being called, no matter what. Those who take heart call do not get any additional pay, other than the fact that because there are fewer of us we take a lot more call- although we are fighting to change that.

We had hearts and neuro specialty teams...yes 30 minutes was required. Hearts and neuro got bonuses ($3000 every 3 months for neuro and a percentage of your current pay for open heart...specialty teams also got $1 more an hour on call).

I recently worked @ a Level 1 trauma center. We had designated call for cardiac, liver, and eye teams each day during the week after day shift ended. As for the rest of the O.R., we were required to sign up for one 13 hour call starting at either 6a or 6p on a Saturday or Sunday based on preference and availability . The option to take more during the four week schedule was available.I like the idea of a Neuro call team as the person above stated their facility offered. I have some experience on the neuro team, but I think having a designated call team in that area would have been helpful in emergent situations as they can be hectic rooms to be in.

Comment:
I should also say we had a 45 minute window from when we were called to being in scrubs ready to go
