To appreciate something can mean to perceive it with the senses; so in this case, he means "no audible S3" (while also tacitly allowing some room for subjectivity, i.e. that someone with better ears might be able to hear one). Comment:
That just means he couldn't hear it. If you can "appreciate" an abnormal assessment finding, it is there, or "appreciable." S3 indicates fluid overload (you probably already knew this) and MD was perhaps evaluating a CHF patient or something similar. In that instance, this would be a relevant assessment finding, if "appreciated."
