med/surg worksheet

Hello everyone!

I was looking through all of the threads re: med/surg worksheet and report sheets. I have not found anything that I like and noticed that these threads have not been updated in a while. Does anyone have a worksheet that they are willing to share? I work on a med/surg unit and can take up to 6 patients.
