Poor Experiences with Electronic Medical Records?

I was wondering if any others have had a bad experience with health data sharing, systems interoperability, or exchange? What issues did it cause for you? I am writing a paper that explores the negative experiences with Electronic Medical Records and possible solutions to combat these issues. Any insight would be greatly appreciated!
