Need a nurse "brain" Neuroscience Telemetry Progressive Care with ENT post-ops too

Hey all!
I'm a new grad nurse just hired on a neuroscience tele progressive care unit, we also have some ENT post-op patients too.

Does anybody have a nurse brain sheet that would help me? I'd like to have time slots (12hr), and mostly be able to circle common options (ie reg diet, soft, cardiac, etc.). I'd also like it organized by body system (head to toe).

Ideally, I'd like it to be a word.doc so I can edit it a bit if I need to!!
