Have there ever been times when a nurse-to-be failed their NCLEX past 4 year period?

Just thought about this today, as some states have a requirement to complete and pass the NCLEX within so and so years after graduation. Theoretically, let's say it's a 45 day period re-take and 4 year time span, that's 8 re-takes in a single calendar year, or 32 re-takes over 4 years.

That's alot of re-takes!
