Could a rural nurse practitioner with a large family take a month off work each year?

Could a rural nurse practitioner with a large family take a month off work each year to go to India to help the poor (the parts nobody wants to go)? This doesn't count time taken off to visit family .

I know this is probably unrealistic, but I just had to ask... If it is possible then how could someone go about achieving this goal?
Maybe - would definitely depend on whether you state this right up front - I would think it might be a reasonable request.

Sure, it's possible, depending upon your employer. Mine was very understanding and time was allowed for things like this. Others, no. Like traumaRUs said, state your intentions up front - try and find an accommodating employer who might be doing the same thing.

Absolutely! As previously stated, make this a condition of your employment up-front.It is not at all unusual for organizations to use locum tenens to enable primary care providers to take time off for conferences, vacations, etc.
