Anyone work as a home health planner or similar position?

I just landed my first position as a home care planner for Metropolitan Jewish Health Systems in NYC. Does anyone have any experience in this field? I will be working directly in various hospitals units meeting with pts and assessing their home care/rehab needs and getting them set up accordingly. I will also be working with their dr.'s and social workers and family members to ascertain discharge instructions are thoroughly explained and understood by the pt. So excited to begin!
