I would wait until you have another job offer before you talk to your current boss. You can always ask the person you interviewed with to wait to contact your current boss until you speak with them yourself first.

You should follow up regarding your transfer. I don't know if you're going to do home health for your hospital or through an agency...but personally I wouldn't tell my boss about anything I was applying for. When you get the job, give him a two weeks notice...done.

I will be leaving my job within the next month for similar reasons and going to a school nurse position. I sat down privately with my boss and explained my reason. I told her I really love my job but night shift is wreaking havoc on my health and my sleep, and that I was looking for a new job. She appreciated the heads up and there hasn't been any kind of retaliation.
