I have several patients with an early stages of dementia and I do a lot of teaching (home health nursing) I have been told to be extra careful when documenting teaching given to a patient with an early stage of dementia who are often forgetful. Thanks
In the LTC I worked at I would document exactly what I taught and the patients reaction, memory of the teaching. I would also state what the patient had problems remembering and how long the teaching time was. Hope that helps.

Whenever possible try to have a family member or responsible party present for the teaching, and document who was there. Also, if possible, have printed handouts for everyone to refer to.I can almost guarantee that there will be little remembered by either of them.......Best wishes!!

Also, in the evaluation section: Patient needs reinforcement.
