Need advice re. job application questions

I'm asking this for a friend, who needs to fill out an application for job.

1. If you finished your probation and orientation and your preceptor signed the paperwork that you're competent to perform on your own, but a few weeks later you are told that this is not a good fit, they don't need you anymore (PRN job), but you get nothing in writing and have not been written up for anything (and the manger offered to help you find a more suitable job, but you didn't follow through on that for various reasons)... how do you answer this question:
1. Have you ever been terminated? If yes, explain.
2. For the above position: Reason for leaving?

The job opening is not in the same hospital. The application is very thorough and also asks for permission for a questionnaire to be sent to previous managers. This includes questions such as "What are [her] strengths, weaknesses, would you recommend [her]?"

Final Question: Would any manager fill out such a reference form, or forward it to HR to deal with?

She could call HR of the hospital and ask if she is eligible for rehire or not. And, since the manager offered to find her another position within the system, she might be able to speak to the manager and ask if she could give her a reference. The worst thing that could happen is the manager saying no.

Thanks for the reply, I'll pass that on.But how should the questions re. reasons for leaving/ever been terminated be answered (based on the facts given, was this a "termination"?) Is there a good way to put this?Thanks again,DeLana
