Care manager Certification New York

Does any one know the Care Manager certification Course for RN in New York, to take this course on site? Please do not spend your valuable time regarding course requirements I am familiar with.
Certification course abbreviation is: CCM.
For sure, it named Care Manager or Case Manager Certification.
If some of you dear members, know location in Manhattan NY, Brooklyn NY or Bronx NY, Queens NY. Manhattan NY-location is very much preferable!
