Need help any info on PCMH care coordinator

Hello to all! I have accepted a new position as a PCMH care coordinator. I was wondering if anyone could give me any information on what the position is like on a day to day basis. I have been nursing for 9 1/2 years, mostly in a youth rehabilitation center and 1 year in a correctional setting. I am nervous, however confident. I was just wondering is this a stressful position? Hello! I came across your comment and wanted to reach out to ask how you have enjoyed your role so far and what feedback you can provide in terms of the work you're doing? I am a PCMH administrator for a medical group and we are in the beginning stages of PCMH, assessing what care coordinators will be needed etc so just curious! Thanks!
