Teaching Learning care plan

Hello everyone. So my teacher has asked us to create a teacher learning care plan with the template. In the template it requires this:

Learning goal: Reflective Journal Writting
cognitive affective pyschomotor

learning objectives


linkage with course outline

resources and strategies


success indicator


target dates


can anyone explain to me how to do this properly? I wanted to do my learning goal on refelective journals. Above is put in a template of how the teacher wants it to be put in. I am just confused on what do to do?
