How to label multiple wound sites from top to bottom?

Hi everyone, I have a patient that has 6 open blister sites on her right lower extremity. One form my company uses has me list wound sites but does not have a diagram to visually pinpoint the wound sites. How would I correctly label each wound from top to bottom? Thank you for your help
Just LOOOVVVE your name!!!Nobody cares for bugs, but I HATE HATE HATE crickets. They just freak me out. And I smoosh 'em!

Comment:
Draw your own diagram and label from top to bottom as you approach them, 1-6, or A-F. I would go through the trouble of doing this on every form that I fill out because you can't count on any other nurse using the same labeling scheme.

Comment:
Why not just draw a little sketch on the side of the form and number them 1-2-3-4-5-6? Put a copy on her chart or at bedside so anybody who has to refer to it will be able to see the same thing.

Lol I don't really smoosh them but I hate them! crickets just give me chills, I don't like how they can be 3 feet away but still have the potential to jump in your face. No!

Sorry my fault, I didn't mention that this is an EMS form and is submitted online.

Do you have the option to photograph wounds? I have had patients in the past with multiple wounds and charting would have taken forever so I take photographs and just chart "multiple blisters to LLE, see photographs".
