looking for examples for SBAR for NICU

Our NICU is developing a new report sheet to help everyone become more organized, and to minimize things forgotten in report. We have to develop a report sheet using the SBAR format (hospital policy). I was just curious to see if anyone has examples from a NICU? Thank you!
