How does your institution record Is and Os/ DWs?

Looking for an effective way to monitor Daily Weights and monitoing input and output on the patients.

What does your institution do? Is it effective?
do you have computer charting? most computer charting will automatically graph it for you

We enter it in the computer and it records it.

Our facility went to computer charting and it would automatically calculate it for you after everything was entered. There was a section for daily weights as well that would sort of show a graph/trend.Before that we wrote all info on the graphic sheet-VS, I/O's, daily wts.
