Decreased Cardiac Output dx will you help?

Decreased cardiac output: Diastolic Dysfuntion R/T altered contractility AEB EFR of 25%, hypotension, hx of CAD, slow capillary refill, anxiety, activity intolerance: fatigue, dyspnea, dizziness; unproductive dry cough, weak peripheral pulses, cool extremities.


Is the evidence overkill? Do I choose just one? My goal is to decrease cardiac workload. A lot of these would be my objective information elaborated.

Thanks. I changed it. I had a panic moment. Seems like every time I do a care plan I forget how to do them.

Are you sure it's diastolic disfunction? This is usually has an elevated EF and BP r/t thickened ventricle. Just sayin'
