Super Morbidly obese patients (1000 lbs)

Does anyone know of a facility, rehab, weight loss center which will take a total care patient near 1000 lbs?
There is a facility on Long Island in New York. Brookhaven. I am not sure where on Long Island. You could Google it. They do take the super morbidly obese.

There's one in Houston, TX but I can't think of the name.

Eagle Point Skilled Nursing and Rehab Center is the one in Ohio, I believe
