on my unit, they do rounds on all pts a few times a week. im new to this...what do they want to hear in report? it should be brief, so im assuming IV meds, but what else about the pts?
Where I work they want a brief report on what they are in with, progress made, barriers to discharge and any referrals needed.We don't discuss meds unless there is an issue.

More about the bigger issues. How is the pt? Quick hx of stay and hilite important happenings. Is there some issue you need help with that one of or all the team can suggest/new orders? Have EMR open esp labs.

I used to do them daily...it was grueling!Started with name, dob, icu day, vent day, post op day....HistoryDiagnosisChanges in the past 24 hours...Airway (ETT size, measurement, last ABG, etc), Breathing (sats, liter flow, etc), Circulation (HR, BP, cardiovascular meds), Neuro (sedation, pain meds, LOC, how many PRN's were given), Electrolytes (most recent, meds), Fluids in/out, gastro (last BM, meds, abdominal assessment), hematology (meds, treats, recent H&H), Infection (tmax, isolation, antibiotics, cultures, pending and resulted), Joints & Skin, Lines, Nutrition (diet, feeds and rates, increasing, decreasing, tpn and lipids, whatever), Plan...I missed a few things that I'm sure will come to me. It sounds like a lot but it became second nature after awhile. Without interruption I could do it in about 3 minutes.
