Nurses and Physical Activity Questionnaire

We would like to invite you to participate in this postgraduate research project. You should only participate if you want to; choosing not to take part will not disadvantage you in any way. Before you decide whether you want to take part, it is important for you to understand why the research is being done and what your participation will involve. Please take time to read the following information carefully and discuss it with others if you wish. Ask us if there is anything that is not clear or if you would like more information.

The aim of the study is to examine the relationships between nurses' physical activity, the attitudes towards physical exercise, and related physical health promotion practice. The results may indicate what factors are more or less important influences on nurses' physical activity, and their health promotion decisions. No financial reward will be offered but a summary of the final report will be available to participants on request.

You will receive a 29-item questionnaire about your attitudes toward physical activity, personal physical activity, and attitude towards physical health promotion practice, promotion of physical health in patients, and perceived barriers to physical health promotion practice, which will take approximately 10 minutes to complete.

Your participation will be confidential and anonymous i.e. no name or number on the questionnaire will make it possible to link information back to you. It will not be possible to withdraw data once the questionnaire has been returned. Your date will only be used in this specific study and stored securely. All questionnaires and other printed documents will be stored in a locked cabinet in the researcher's office. Electronic data in word/excel format with a password to open will be saved on a USB stored together with the printed documents. The researcher, her supervisor and the statistician are the only people who will have access to the collected data.
