Question about injection documentation

Im an LPN and new to clinic setting. We use practice fusion as our EMR. I know that I need to doc. med,strength, dosage, route, and site. It is necessary to doc. lot number and expiration from vial?
if it is a vaccine then yes, it is necessary to document the lot number, manufacterer and other accompanied information. (:

In addition to vaccines, you often will need to document it for PPDs as well.Otherwise...unless your facility's policy specifies otherwise, you shouldn't have to document lot and expiration.

at this level, i would address my concerns by reviewing the facility manual policy procedure which could differ from other facilities, in order to be sure. wishing you a great week....aloha~
