Hello, our names are Daniel Mead and Domonique Mathis. We are both Master's degree in nursing candidates as DePaul University in Chicago, IL. We are conducting a research study as part of our program under the supervision of our research advisor, Dr. Matthew Sorenson ([email protected]) from the School of Nursing at DePaul University.

We will be examining what types of fresher education, if any, hospitals and medical facilities use to help keep registered nurses' proficiency of Cardiopulmonary Resuscitation skills. This study is open to anyone who is a registered nurse, has BLS certification, currently employed in a healthcare setting, currently providing patient care and is not a critical or acute care nurse or a nurse that holds ACLS certification.

If you agree to participate in this study, completing the survey at Qualtrics.com will indicate your consent to participate in the study. The online questionnaire consists of 16 questions, some multiple choice and some short answer. The survey should take about 10-15 minutes to complete pending on the length of your responses. There will be no identifying information collected so the questionnaire is completely anonymous. At DePaul University, study results will be analyzed and discussed in a research paper. Once the article is completed, it will be available to you upon request.

We are conducting this research project because we are interested in the need to refresh Basic Life Saver Skills (mainly Cardiopulmonary Resuscitation skills) in the healthcare setting. We are asking you to be in this research study because we are trying to learn more about what strategies are used to help registered nurses retain and remain proficient in cardiopulmonary resuscitation skills. We are asking you to be in this research because you are a registered nurse in a healthcare setting, are BLS certified, and provide direct patient care. You cannot qualify if you are a critical care nurse, acute care nurse, or are ACLS certified.

If you agree to participate in this study, you will be directed to a 16-item questionnaire located on Qualtrics.com. This questionnaire will not ask any identifiable personal data, only general demographic data, so your anonymity will remain intact throughout the process. Please answer every question as truthfully as possible, we really do value your opinion. If there are any questions you do not feel comfortable answering, please feel free to skip them.

This questionnaire should take 10-15 minutes to complete. Once again, you anonymity will be protected. You have the right to withdraw your participation at any time. There will be no negative consequences if you choose to not participate or if you choose to withdraw your participation. You will be able to exit the questionnaire at any time. However, if you choose to submit your data, you will be unable to withdraw it due to the anonymity of the study.
