I think Briggs catalog has a form ( or more) that NHs use. I know we've used pre-printed ones at the various facilities that I've worked.Briggs has all kinds of forms.

WE used the Morse Fall Scale, I don't remember if it includes vital signs, we would monitor them on a separate form.
