PCMH Care Coordinator Update

Hi to all! I have been an LPN for 9 1/2 years, working in a youth rehabilitation center for 8 1/2 and correctional nursing for one year. I recently accepted a position as a PCMH care coordinator in an ambulatory setting, which is affiliated by a large state hospital. I am nervous, however, confident and a fast learner. I was wondering if anyone experienced within this field/position could provide me with any details of what it's like and how your day goes? Is this a stressful position?
