Im in Illinois--- lost PRC license card

pls help... im recently in illinois and decided to start processing my nclex application. i lost my prc license card here,,, i believe you would need a copy of it for the processing in PRC back in the philippines. my questions are:
• can i send a copy of my board certificate instead of the license card? will PRC consider it?
• if not, how am i going to processto get a duplicate of my license card???
i checked the PRC website and says i have to have a notarized affidavit of loss,,, do i have to get it here in illinois and send it to a representative back in the philippines together with an authorization ketter and ID picture???

any suggestions and clarifications are greatly appreciated. thank you.
