Need assistance on how to properly respond to possible consular inquiry

My friend who has been to Saipan (U.S. Territory) and is planning on visiting the United States for a convention is wondering how to properly answer the question "Have you ever been to the U.S.?".

Is a visitors entry permit considered a visa?

What visa did she need for entering Saipan ? Did she get the visa from the US Embassy? Was it the same visa that one would use to enter the US or a different visa ? Did the visa only say Saipan ?
