care manager Ohio

I have been offered a position from a private company affiliated with Humana as a care manager. This position is paid flat rate per each client seen in person each week. Just wondering how others like this position and what is their take of this job?Do you like it is the job manageable etc... I have read some of the full time positions can be huge over time and are paid salary only , but the flat rate role could be different. Thanks
