Erm...that's a whole lot of personal information to be posting online.Not sure if that is your whole resume but more than likely it will be filtered through a web system set to look for key words, so it would be more likely to get into the system if you identify specific skills and talents associated with the job you are looking for and then find a way to expand on your job experience by using those words.Sort of like a backward google search.Just saying your job title and where you have worked isn't going to get you very far, you're competing with a gazillion other people. Best to lay out the specifics of your duties and resposnibilities, in bullets or some similar format.There are some good sites online for sample resumes.

Should your job description be in the past tense? "Assisted residents" or "provided". Also agree with above, latest stuff first. Above all else, it should be easy and fast to read.

It looks pretty good to me, just put everything in order of most recent first. Did you do anything "extra"? It looks like you just showed up and didn't get fired (not a bad thing). Did you create any job aids, training tools, write any policies, write any articles, attend conferences, get awards, serve on a committee? I'm saying it looks real solid (I especially like the PALS Instrutor) but where's the bling? You must have done something more, write it down. Don't avoid "showing off". You want to blow away the competition so you can get a job. Good luck.
