Is it possible for a pt to ejaculate with a foley in?

The A&O patient is claiming he has a BM and when you go to clean him up, it looks exactly like semen. Multiple staff have seen it and they all agree. However, he has a foley in and the MD says its impossible, but I've honestly never seen clear-to-white sticky BM, no BM smell. Especially, "BM" thats more on the patients penis and front of his testicles than his rear end.
I never actually thought about it before. I do recall doing peri care on a man once that had a penile implant, that was interesting.

i would deff like to know the answer to this...the other night i was bathing my pt and there was what looked like semen around the top of his penis and foley... :icon_roll

By the way, this patient also was dx'd with UTI a few days ago and the UA says theres spermatozoa in the urine. Which I didnt think much of it, but when I saw what was on him today, I seriously almost puked. Another aide today told me he also likes to flirt, if thats really what it is.

I've never personally seen it, but a nurse did tell me she had a patient do it once!

Also, this patient is HIV+.... Is there anyway a "clear" BM be a complication or something of HIV (e.g. increased suspecibility to infection --> stool infection???)? Is there anything that can cause a clear BM? I've seen mucusy stools, but nothing completely clear-white and mucusy

Ejaculate has a distinctive smell, one you've smelled it you should be able to distinguish it from BM that looks like ejaculate.

While I don't know for sure, I would imagine it would be possible. If you think about the anatomy, the seminal fluid enters the urethra after were it is joined with the bladder. So if ejaculation does occur while catheterized, with the required force it would stretch the urethra and/or collapse the foley enough to escape. Also with the foley in place, that leaves an opening to the bladder where it could also go and mix with urine to leave as output in the foley. It makes sense to me especially since I have seen plenty of foleys leak.

I've actually seen patient info leaflets with 'how to' with a catheter in! so I guess it can be done.

How old is the patient? I assume not 80...
