I'm not familiar with everything that's included in this situation, but at least some of it is required by JCAHO, the same as if you were a permanent employee.

If it was me I certainly would ask my boss and want to see what it is that is faxed.

nothing wrong in asking. We require license # (although we check online for status), CPR and other certs, work history, drug screen and background screen results, TB and vaccine info and competencies. If JC comes by and traces a patient you cared for we need to be able to show the same data as if you were an employee. We do not do credit checks.
