The best abbreviation for medical charting is NO abbreviation. I've never seen qpm either & I have almost 37 years as an RN.

Some of the MD's in my office use it but I'm not sure if it is an accepted abbreviation.

i don't remember what medication it was, but it's how the doc wrote the order so i just copied it to the paper MAR. I wonder if we'll get dinged for that. It's so annoying too because i never see orders written that way, it was a doc i don't usually work with. ugh

I've never seen it either - and my job was auditing MARS and orders. I see qAM or qXXX time if other than AM or HS.

Your resource should be your facilities P&P. I wouldn't sweat it at this point, however if you are asked a question by a JC surveyor you could just point to your references (P&P) that you have available. You can't know everything, but you do need to know where to get your information.

I was thinking "q" anything isn't to be used anymore. But out computerized entry system has a "qpm 21".But I would suppose you could do a "qpm cc" for basically, "with dinner," makes sense to me

QPM- Questions per minute from the admin types, like JCAHO. Not to exceed1-2 for more than an hour. Yep, that's it.
