hi ,

i need to d[font="arial"]escribe in detail an example of an intervention ihave undertaken with a patient / client with complex needs. i dont really understand this. can anyone give me some advice? it would be in the elderly age group. my mind has gone blank. i dont really get it....
When in doubt, fall back on Nursing process. Start off by describing some information you gathered (assess) that indicated the patient had a problem - e.g., difficulty reading instructions due to vision problems. Then, describe what options you thought about (plan) and why you did them... e.g., cleaning glasses, obtaining instructions with larger type font, etc. Try them (intervention) Then finish up by evaluating your intervention.. e.g., asking the patient to read the new set of instructions and asking him if it is working better for him.Your instructors are probably checking to see whether you have internalized the whole nursing process thing. Don't get fancy - keep it well within your realm of understanding and with low-level interventions that are definitely within your own scope of competency. Good luck
