Individualizing Care Plans within an EMR

Greetings, all! ---

Would anyone have a resource for guidance on individualizing care plans within the framework of EMRs? EMRs are so canned & constricted (at least, ours is --- we have limited text fields) that I am concerned about individualizing the care plans (for swing/skilled patients) enough to satisfy the newly refocused surveyor scrutiny. Would really like to find resources with suggestions on how to reconcile these two needs.
