New Field of Nursing: Population Care Coordinator Nurse

I have recently been hired to work as a Population Care Coordinator Nurse at a medical practice/Dr's office. Is there anyone who happens to be a PCC? This is definitely a new field of nursing in healthcare. Can you please describe or share what responsibilities or tasks you would do on a typical day?

I read many research articles and publications on the Internet about PCCs. The one thing I might add is that I got this job through a connection. I am a new grad who hasn't found a job within over a year of graduating and I got hired by the practice. I have no hospital experience outside of nursing school and I suppose a PCC should have that experienced clinical background and hospital experience, but I don't. I am so interested in this job and I want to really succeed and excel in this job, even though I am not quite sure about the job description, since it's so new and unlike anything I've heard about.

I know that PCCs are being hired to work in primary care offices as part of a project with the insurance company of Horizon BCBS, to transform the way patients are cared for, while transforming your typical "doctor's office" into a "Patient-Centered Medical Home."

Do you think it's a bad idea to go through with this job as a new grad? This is my first big break and I know that this will really beef up my resume in the future years to come. I really want to excel in this job, no matter how long it takes!
I had to do some research myself as I had not heard of this nursing role. I have to admit that I am very confused, given the nature of the position, that you would be hired as a new RN.I hope that you will not take that as an insult. I am more concerned about you, as a new nurse, being placed in a position where you could potentially sink rather than swim. You are right to seek as much advice as you can.I totally understand your desire to take this opportunity on this tough employment market.
