Physician's Wish List in Nursing Homes

At the facility where I work, when one of our residents has an issue that doesn't merit an immediate call to the clinic, we put them on a "wish list" with name and a note about the issue. Different providers come a couple of times a week. This is a very complicated and cumbersome procedure that we are looking to simplify, along the lines of Lean-6. I have been trying to research for a more efficient process, but so far have not been able to find any articles on the subject. Any suggestions for articles to read, search terms, description of a successful process, etc, would be greatly appreciated. Thanks in advance dbbik
