Collaborative Care Nursing Model

Has anyone ever heard of Collaborative Care Nursing?? If you have heard of it, what all does it involve?? We are going to be using this nursing model soon. I was just curious if anyone else is using this or has heard of it. If you are using it, what are the pros and cons, and how is it working for your hospital?
